Healthcare Consultants . (FLI) is pleased to offer you this Electronic Health Record (EHR) service. Our service allows you to enter you personal history and current concerns so that they can be reviewed by staff even before you set foot in the door. However, please familiarize yourself with the terms of use as you are bound by them if you choose to use the system. You are not required to utilize this EHR. You may complete the registration form by clicking the link below and printing a PDF copy our our intake. Please complete the form and be sure to bring it with you to you scheduled intake appoitment.

Below you will find information about how you can access your data from any web-based computer to update your information or to create additional profiles for other members of your family. All members who utilize the EHR system will have to create a username and password. Your username will be the e-mail address that you use to initially request an authorization to create an account which will be done by determining that you are a FLI patient. Once confirmed, you will receive a login email with instructions needed to register for yourself. Only confirmed parties will be allowed access to the system. Please have the date and time of your appointments that we may validate your account. While the online questionnaire is lengthily, it allows FLI to gather information that may be overlooked by others. You can always save your information, using the save button at the bottom of the assessment, and return to the assessment to complete it at another time. We hope that you enjoy using the EHR system.

Please read the terms and conditions entirely and carefully before using FLI EHR system. Again you are bound to the terms of this user agreement. These terms and conditions are subject to change without prior written notice at any time at Healthcare Consultants .’s sole discretion.

FLI thinks that it is important for you to know how we manage information via the Internet. This Terms & Conditions Statement outlines our practices and sensitivity to your right to privacy. We reserve the right and discretion to revoke access to this service at any time.

Electronic Communication

FLI does not at this time use this system as a way of communicating directly with FLI staff. Appointments and emergencies still need to be addressed by office staff, including after-hours coverage. Emergency situations requiring immediate attention from FLI staff should not be submitted electronically. All medical emergencies should present at a hospital or call 911.

FLI does offer links to related medical and informational Web sites not managed by FLI. These Web site link(s) are for a patient's informational purposes only. FLI does not endorse and has not verified the accuracy of the information in/on these Web sites, and the patient should not rely on any of the information found on the Web sites for purposes of treatment or diagnosis. Information provided by your physician should not be rejected or changed due to information you gather from another website. Doing so may put your health or the health of your child in serious jeopardy.

Minor Eligibility

An individual must be age 18 or older to create an electronic FLI account.

Email Privacy

Patients who are users of FLI EHR system should be aware that they will be notified by email for certain features, including activation. This means that any person with access to a patient's email will be able to see these types of notification(s). This could include the patient's spouse, employer or anyone else that can access a patient's email account. Some computers contain software that records keystrokes and screen shots which could include your Private Health Information (PHI) and/or your username and password. It is always best to access FLI’s EHR system from a trusted computer. FLI uses encrypted transmission for all PHI based Internet activities to protect your sensitive information. A patient's confidential medical information related to FLI features will be accessible only to appropriate Healthcare Consultants . staff.

Security and Confidentiality

We afford the same degree of confidentiality to medical information stored on FLI’s EHR as is given to medical information stored by FLI in any other medium. FLI is committed to protecting the confidentiality of your medical information. We limit FLI employees and agents’ access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

For other than general information viewing, FLI’s EHR system must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (the latest free additions of Netscape, Mozilla, Safari or Internet Explorer will all work). Our SSL web server’s authentication protocol offers the highest level of encryption technology commercially available (128-bit) through Geotrust; a highly respected leader in the industry.

You can tell when you are in the secure portion of the site by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to FLI by viewing the digital certificate. This certificate verifies the connection between the FLI server's public key and the server's identification. The certificate may list “secure.brentphd.com” as the certificate holder.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a FLI user, your role in maintaining the security of your medical information is enhanced by keeping your login and password private and reporting any changes of your data that you did make. By law we are required to back up our database each month in an encrypted format. Also by law, we are required to notify you within 30 days if we believe that our EHR system has been breached. You also have the right to close you EHR record. This must be done in writing and may take 30-60 to process.

The site is in complainace with the security standards that are set forth by HIPAA and HITECH. However, updates to the laws do take place form time to time. Any nessicary changes that effect our patients will be provide to them in writting as per HIPAA and HITECH statues.

Your FLI EHR account may be deactivated due to specific terms and conditions for use of the applications at the discretion of FLI. These included no longer being a patient for the practice/physician, inappropriate use of scheduling, demands that have describe as not the function of the website (emergencies), abuse of proxy access, or pending legal issues with patient or direct family. This list is not considered to be all-inclusive and additional guidance can be addressed with a written request. Again, request may take between 30 and 60 days to be answered.
